Powered pressure mattresses divide broadly into two approaches, and knowing which one you are specifying matters more than the brand on the pump. **Alternating** systems cycle cells so that tissue is completely offloaded on a rotation — a cell under the sacrum deflates entirely, blood flows back into that tissue, then it reinflates and its neighbour deflates. The client is never unloaded everywhere, but every area is fully unloaded periodically. This is the approach with the strongest rationale for treating an established wound. **Constant low pressure / immersion** systems instead hold a uniformly low pressure and let the client sink in, spreading load across the largest possible contact area. Nothing is fully offloaded, but nothing is heavily loaded either. This tends to be better tolerated — no cycling to feel, no movement to disturb sleep — and better suited to a client with pain or with a condition that makes cell movement uncomfortable. Many systems, including the Zephair range, offer both modes. Choosing between them for a specific client, rather than accepting the default setting, is a genuine clinical decision and one worth documenting in a submission. Three things to check on any powered system at every review, because these are where they fail silently: • **The weight setting.** A pump left on the previous client's setting either bottoms the current client out or holds them too rigid to immerse. Neither is visible from the doorway. • **Bottoming out.** Slide a flat hand, palm up, under the sacrum with the client on the surface. If you can feel the base through the cells, the mattress is not working where it matters most. • **Layers.** Every additional sheet or protector between the client and the surface reduces immersion. One layer, stretchy, flat. Confirm the current model, mode options, capacity and Australian supply arrangements before quoting.
