Cushion selection comes down to a trade-off that no single material resolves, and it is worth having the framework straight before looking at any specific product. **Foam** is light, stable and gives a firm base to transfer from and to hold the pelvis. It is the weakest of the materials at pressure redistribution and it degrades — a foam cushion two years old is not the cushion that was prescribed. **Gel** conforms closely, spreads load over a large contact area and damps vibration well. It is heavy, and a sealed gel pad can migrate away from a bony point over time and stop working there. **Air** gives the best immersion and the best pressure performance, because the client sinks in and load spreads across the largest possible area. The costs are stability — an air cushion is a poor base to transfer from — and maintenance, since inflation has to be checked and a puncture is a total failure. **Hybrid** combines a foam or contoured base with a gel or air insert over the ischial region. Adequate at both jobs rather than excellent at either, which is what most clients actually need. The honest rule is that no cushion compensates for a poor seated posture. If the client is sacral sitting — pelvis rotated backwards, sliding forward, weight through the coccyx and sacrum rather than the ischial tuberosities — then the cushion is not the problem and no cushion will fix it. Seat depth, seat-to-back angle, footplate height and back support have to be right first. A high-specification cushion under a collapsed posture is money spent on the wrong thing.
